Vision Report Card© Instructions
Step 1:
School nurses make photocopies of the Vision Report Card© using the master copy provided to them by the School Nurses Association (or available at www.nechildrensvision.org). Blank copies are distributed by the school nurse to classroom teachers.
Step 2:
A classroom teacher and/or school nurse or other designated school official documents her/his observations on the Vision Report Card©. Two photocopies are made of the completed Vision Report Card©. The school nurse keeps one original report card for her/his records. (Note: The school nurse should receive the completed forms, if other school officials complete the Vision Report Card©.)
Step 3:
The two photocopies of the Vision Report Card© are then forwarded by the school nurse to the parents. One copy is for the parents to keep and one copy is for the parents to give to the eye care professional. This will encourage parents to take their child, plus the Vision Report Card©, to the eye care professional of their choice for an examination.
Step 4:
The parents take their child along with a copy of the completed Vision Report Card©to an eye care professional. The completed Vision Report Card© helps the eye care professional know if there are specific symptoms or complaints that should be evaluated.
Step 5:
The eye care professional documents her/his examination findings on the Professional Vision Report©. The eye care professional keeps one copy and makes two photocopies of the completed Professional Vision Report©. One copy should be given to the parents and the other copy should be sent to the school nurse by the eye care professional after obtaining proper parental approval, so that the school officials know that the child’s vision has been evaluated.
Step 6:
The school nurse then provides a copy of the Professional Vision Report© to the classroom teacher(s) so that educators are aware of any pertinent findings.
Step 7:
The classroom teacher(s) reviews the Professional Vision Report and makes instructional accommodations as suggested, if possible. All interested individuals continue to monitor student achievement. Further communication related to the student’s vision and learning may be initiated at any time by parents, teachers, school nurses, etc.
